REGISTRATION

Registration fees for tackle football are $350 until April 1st, 2011. After April 1st registration will be $400. Registration for flag football is $175.

THERE ARE 4 STEPS TO COMPLETE SEAHAWK REGISTRATION:

1. Registration Packet:
There are 2 ways to complete your registration packet for the 2011 Seahawk season. The first would be to download, print and fill out the entire packet and bring to one of the Registration events that will be held at DICK’S SPORTING GOODS in Fashion Island on MARCH 2nd, 9th and 16th. Registration packets will also be available at DICK'S on those dates.

 

COME TO DICK'S SPORTING GOODS MARCH 2nd, 9th or 16th to REGISTER FOR the 2011 SEASON!!


If you are unable to attend any of the Registration events at DICK’S, then download this packet and MAIL COMPLETED and SIGNED (keeping a copy for yourself would be a good idea) Registration packet to this address: 1087 Irvine Avenue #715, Newport Beach, CA 92663. If you are mailing a registration packet after April 1st, 2011, please email nmjaafseahawks@yahoo.com to let us know your registration is in the mail.

 

2. Payment:
Payments can be made by cash, check or Paypal (Paypal accepts debit and credit cards as well as payments via checking account).

 

3. Physical:
The Newport-Mesa Seahawks will hold a 1-day Physical event in Spring that includes a free physical. If you are unable to make the Seahawk Physical event, you are responsible for getting a physical completed and signed by a qualified doctor on an official OCJAAF (Orange County Junior All-American) Contract. If you need to get your OCJAAF contract in order to have your physical done, please email our player agent at nmjaafseahawks@yahoo.com to make arrangements.

 

4. Proper Documentation:

BRING PROPER DOCUMENT TO THE PHYSICAL EVENT:


ALL PLAYERS MUST BRING PROOF OF RESIDENCE i.e. current utility bill, tax bill, bank statement, etc.

ALL FIRST TIME PLAYERS MUST ALSO BRING ONE OF THE FOLLOWING to PHYSICAL
(1) Original or certified copy, with red stamp, from a city, municipality, county or state
(2) original passport or immigration visa (3) a U.S. government birth certificate OR (4)
original adoption papers wherein the player’s name and date of birth are properly recorded.
HOSPITAL BIRTH RECORDS are NOT ACCEPTED.

IF YOU ARE UNABLE TO BRING PROPER DOCUMENT TO THE PHYSICAL EVENT:
then once the season starts you will need to leave your proper documentation with
your team admin (team mom) until an OCJAAF official is able to sign off on your documents
in order for your player to be on the official team roster.

One last thing, if you own a Seahawk helmet and plan to use it for the 2011 season, and it has a certification date of 2009 or before, it must be re-certified/reconditioned per OCJAAF rules. Or If you own a helmet that you'd like repainted for the 2011 season, it also must be re-certified. Please bring any helmet that requires re-certification with you to the Physical event at the Newport Sports Museum or contact our Equipment Manager. For $27 we can send your helmet in for painting and/or recertification. All helmets must be certified every 2 years.
